A recent NY publication commented on the versatility and utility of  the VA system  for computer records  ( ViSTa). The association  now suggests that using  computer records is better for patient safety.  For all of us who have conducted retrospective reviews and encountered missing charts or documentation, we know how frustrating paper records can be.  Why then  do paper records still persist in many  hospitals ?  Might it become a JCAHO requirement to have computer records?
